Open Wound of the Lip: First Aid, Healing and When to Get Stitches

An open wound of the lip is a cut, split or tear in the lip, usually from a fall, a blow to the face, a bite or a sharp object. Most small lip cuts stop bleeding with 10 minutes of firm pressure and heal in 1 to 2 weeks with simple care. A cut that crosses the border between the red lip and the skin, goes through the lip, or gapes open needs stitches within hours to heal well and avoid a visible step in the lip line.

Also called: Lip laceration, cut lip, split lip, lip injury, lip trauma

At a glance

What it is
A cut, split or tear in the lip, inside or outside.
Common causes
Falls, sports, punches, biting the lip, dog bites, sharp objects.
First aid
Firm pressure with clean gauze for 10 minutes, then cold to reduce swelling.
Needs stitches if
It crosses the lip border, goes through the lip, gapes, or is longer than about 1 to 2 cm (half an inch).
Healing time
5 to 14 days; the inner lip heals fastest.
Main risks
Infection, a notched scar, and missed tooth injuries.

When to get help

Call 911 or go to the ER

  • Bleeding that will not stop after 15 minutes of firm pressure, or blood pooling in the throat
  • Trouble breathing, swallowing or speaking after a facial injury
  • A broken jaw (teeth that no longer meet, inability to open the mouth), a knocked-out permanent tooth, or a head injury with confusion, vomiting or passing out

See a doctor within a day or two

  • A cut through the full thickness of the lip, or across the line where the red lip meets the skin (vermilion border)
  • A gaping wound, flap of tissue, or a cut longer than 1 to 2 cm (half an inch): stitches work best within 6 to 12 hours
  • Any animal or human bite to the lip
  • Dirt, glass or a tooth fragment stuck in the wound
  • Increasing redness, pus, swelling or fever above 38 °C (100.4 °F) in the days after

Book a routine appointment

  • A loose, chipped or painful tooth after the injury
  • Tetanus vaccination more than 5 years ago for a dirty wound, or 10 years for a clean one
  • A lip cut that has not healed after 2 weeks

Usually safe to manage at home

  • A small cut or split inside the lip that stopped bleeding with pressure
  • A shallow split on the outer lip that stays closed when you relax your mouth

First aid for a cut lip

  1. Wash your hands and rinse the mouth gently with water to see the wound.
  2. Press firmly with clean gauze or a cloth for a full 10 minutes without peeking. For a cut inside the lip, pinch the lip between gauze on both sides. Lips have a rich blood supply, so they bleed heavily at first, but most bleeding stops with pressure.
  3. Check the teeth. Look for chipped, loose or missing teeth. Keep a knocked-out permanent tooth in milk or saliva and see a dentist within 30 to 60 minutes.
  4. Apply cold wrapped in a cloth, or suck on ice or a frozen pop, for 10 to 15 minutes at a time to limit swelling.
  5. Decide if it needs stitches (see below). If so, cover it and go to urgent care or the ER.

Does a lip cut need stitches?

Usually fine without stitchesNeeds medical repair
Small cut inside the lip or mouth under about 1 cmCuts that cross the vermilion border (red lip to skin)
Edges stay together when the mouth is relaxedWound goes all the way through the lip
Shallow outer-lip splitGaping wound, flap or missing tissue
Bleeding stopped with pressureCuts longer than 1 to 2 cm (half an inch)
No bite or dirt involvedBites, dirty wounds or debris inside

The vermilion border matters most. Even a 1 mm misalignment of this line is visible from a conversational distance once healed, so a clinician lines it up with the first stitch. For this reason, lip cuts crossing the border are not closed with skin glue or strips at home. Cuts on the skin above or below the lip are also hard to keep closed with strips because the mouth moves constantly.

What happens at urgent care or the hospital

A doctor will clean the wound thoroughly and look for teeth fragments or debris, sometimes with an X-ray if a tooth is broken and a piece is missing. They usually numb the area with a local anesthetic injection or a nerve block, which avoids distorting the lip shape.

Repair depends on the layer injured:

  • Inner lip (mucosa): absorbable stitches that dissolve in 1 to 2 weeks. Small inner cuts are often left to heal on their own.
  • Muscle: deep cuts through the muscle ring around the mouth are stitched in layers so the lip moves normally and does not form a dent.
  • Outer skin and red lip: fine stitches, aligning the vermilion border first. Non-dissolving skin stitches are typically removed after about 5 days.

Bites are cleaned especially well. Antibiotics are often given for human and animal bites and for wounds through the full thickness of the lip, because mouth bacteria can cause infection. A tetanus booster is given if you are not up to date. Children sometimes need sedation to keep still for an accurate repair.

How to heal an open wound on the lip

For the first 48 hours

  • Use cold compresses to reduce swelling.
  • Eat soft, cool foods. Avoid salty, spicy, acidic and crunchy foods that sting or reopen the wound.
  • Drink through a straw only if it does not hurt.

Keeping it clean

  • Inside the lip: rinse with warm salt water (half a teaspoon of salt in a cup of water) after meals, 3 to 4 times a day. Inner cuts turn white or yellow as they heal. That is normal healing tissue, not pus.
  • Outside the lip: wash gently with soap and water twice a day and pat dry. Keep it covered with a thin layer of petroleum jelly. A moist wound heals faster and scars less than one that dries and cracks.

Things that slow healing

  • Licking, picking or biting the wound.
  • Smoking and vaping, which reduce blood flow.
  • Stretching the mouth wide, for example with large bites, for the first week.

Pain relief

Acetaminophen (paracetamol) or ibuprofen can help for a few days. Follow the label dose. Do not give aspirin to children or teenagers.

How long a lip cut takes to heal

Type of woundTypical healing
Small cut inside the lip3 to 7 days
Split lip on the outer surface7 to 10 days
Stitched full-thickness lacerationSkin closed in 1 to 2 weeks; scar matures over 6 to 12 months

Complications to watch for

  • Infection: spreading redness, warmth, throbbing pain, pus or fever after 2 to 3 days. Infection can spread into the skin as cellulitis.
  • A notched or stepped scar: usually from a misaligned border. A scar revision can correct it later.
  • A lump inside the lip: a mucous cyst (mucocele) can form if a small saliva gland is damaged. It often resolves, but persistent ones are removed.
  • Missed injuries: dental trauma and a broken tooth are common with lip cuts. A tooth fragment embedded in the lip can cause infection.

Scarring

Most lip wounds heal with a fine line. To reduce scarring, keep the wound moist while it heals, protect the healed area with SPF lip balm for 6 to 12 months because sun darkens new scars, and gently massage the scar once it is fully closed. If a scar is raised, tight or uneven after a year, a plastic surgeon or dermatologist can advise on options.

Lip wounds in children

Children often cut the lip on their own teeth in falls. Check for a torn frenulum (the small fold of tissue between the upper lip and gum): it bleeds a lot but usually heals without stitches. A torn frenulum in a baby who is not yet walking can be a sign of non-accidental injury, so doctors take it seriously.

Frequently asked questions

How do I heal an open wound on my lip quickly?

Keep it clean and moist. Rinse inner cuts with salt water after meals, and cover outer cuts with a thin layer of petroleum jelly. Avoid licking, picking, smoking and salty or spicy food. Most small lip wounds heal in 5 to 10 days.

When should I go to the hospital for a lip cut?

Go to urgent care or the ER if the cut crosses the edge of the red lip, goes through the lip, gapes open, is longer than about 1 to 2 cm, was caused by a bite, or keeps bleeding after 15 minutes of pressure. Stitches are best done within hours.

How do I treat an open wound on the side of my lip?

A crack or split at the corner of the mouth heals best if kept moist with petroleum jelly and protected from stretching. If it keeps reopening or does not heal in 2 weeks, it may be angular cheilitis, a yeast or bacterial infection, and needs treatment.

Why is my lip wound white?

A white or yellowish film over a wound inside the lip is normal healing tissue. Inside the mouth, wounds do not form a dry scab. Worry only if pain, swelling and redness are increasing or you have a fever, which suggests infection.

Can you put skin glue on a cut lip?

Skin glue can close small, straight cuts on the skin around the lip, but it is not used on the red lip, inside the mouth, or across the lip border. Those areas are moist and move constantly, and precise alignment with stitches gives a better result.
