Pemphigus Vulgaris

Pemphigus vulgaris is a rare autoimmune blistering disease in which antibodies attack proteins that hold skin cells together. Without treatment it can be fatal; modern therapies are effective.

Features

  • Painful, easily-broken blisters and erosions
  • Mouth involvement often appears first
  • Genital, scalp, and skin involvement
  • Nikolsky sign (skin separates with rubbing)

Treatment

  • Corticosteroids (initial control)
  • Rituximab (now first-line for many patients)
  • Immunosuppressants (mycophenolate, azathioprine)
  • IVIG and plasmapheresis in severe cases
  • Wound care, nutritional support
