Platelet Function Disorders

Platelet function disorders cause bleeding because platelets don't work properly, even if their numbers are normal. Causes can be inherited (von Willebrand disease, Glanzmann's) or acquired (medications, kidney disease).

Causes

  • Von Willebrand disease (most common inherited bleeding disorder)
  • Glanzmann thrombasthenia, Bernard-Soulier (rare inherited)
  • Aspirin and NSAIDs
  • Kidney disease (uremia)
  • Some blood cancers

Treatment

  • Desmopressin (DDAVP) for von Willebrand and others
  • Antifibrinolytics (tranexamic acid)
  • Platelet transfusion for severe bleeding or before procedures
  • Stop offending medications
  • Treat underlying cause (e.g., dialysis improves uremic platelet dysfunction)
