Piriformis Syndrome

Piriformis syndrome is irritation of the sciatic nerve by the piriformis muscle in the buttock. It causes deep buttock pain and can mimic sciatica from a herniated disc.

Symptoms

  • Deep, aching buttock pain
  • Pain radiating down the back of the leg
  • Tingling or numbness in the buttock and leg
  • Pain worse with prolonged sitting, stair climbing, or running
  • Tenderness over the piriformis muscle

Treatment

  • Physical therapy with stretching and strengthening
  • Activity modification
  • NSAIDs
  • Heat/cold therapy
  • Trigger point injections
  • Botulinum toxin injection in refractory cases
  • Surgery rarely needed
