Low Testosterone

Low testosterone is a clinical and biochemical syndrome in men with persistently low blood testosterone plus characteristic symptoms. Treatment can improve symptoms but is not appropriate for everyone with low levels.

Symptoms

  • Reduced libido
  • Erectile dysfunction
  • Decreased energy
  • Depressed mood
  • Reduced muscle mass and strength
  • Increased body fat
  • Reduced body and facial hair
  • Decreased bone density

Diagnosis

Two morning total testosterone levels confirmed low, plus consistent symptoms. Additional workup looks for cause (pituitary problems, medications, obesity, sleep apnea).

Treatment

  • Address contributors (obesity, sleep apnea, medications, opioids, alcohol)
  • Testosterone replacement therapy when appropriate (injections, gels, patches, pellets)
  • Discuss fertility implications before starting (TRT suppresses sperm production)
  • Monitor PSA, hematocrit during treatment
