Hypoparathyroidism

Hypoparathyroidism is a condition in which the parathyroid glands produce too little parathyroid hormone (PTH), leading to low blood calcium and high phosphate. Most cases occur after neck surgery.

Symptoms

  • Muscle cramps or spasms
  • Tingling around the mouth or in fingers/toes
  • Chvostek's and Trousseau's signs on exam
  • Severe — seizures, laryngospasm, cardiac arrhythmias
  • Dry skin, brittle nails, dental problems
  • Mood changes

Treatment

  • Calcium supplementation
  • Active vitamin D (calcitriol)
  • Magnesium replacement when low
  • Thiazide diuretics in selected cases
  • Recombinant PTH for severe refractory cases
  • Monitor calcium, phosphate, kidney function
