Vocal Cord Dysfunction

Vocal cord dysfunction (VCD), also called paradoxical vocal fold motion, occurs when the vocal cords close instead of opening during breathing. Symptoms mimic asthma but don't respond to typical asthma treatments.

Symptoms

  • Sudden difficulty breathing
  • Stridor (high-pitched sound on inhalation)
  • Tight throat or chest
  • Hoarse voice during episodes
  • Triggered by exercise, strong odors, stress, reflux

Diagnosis

  • Laryngoscopy during symptoms (definitive)
  • Lung function tests (often normal between episodes)
  • Exclude asthma and other causes

Treatment

  • Speech-language pathologist for breathing retraining
  • Manage triggers (reflux treatment, allergy control)
  • Address anxiety
  • Reserve asthma medications for documented coexisting asthma
