Pressure Ulcers (Bedsores)

Pressure ulcers — also called bedsores or pressure injuries — are areas of skin and tissue damage from sustained pressure. They commonly affect people who are immobile and can cause serious infection if not prevented or treated early.

Stages

  • Stage 1 — intact skin with non-blanchable redness
  • Stage 2 — partial-thickness skin loss; shallow open ulcer
  • Stage 3 — full-thickness skin loss with visible fat
  • Stage 4 — full-thickness with exposed bone, tendon, or muscle
  • Unstageable / deep tissue injury — eschar or persistent purple skin

Prevention

  • Reposition every 1–2 hours in bed, every 15–30 minutes in a wheelchair
  • Pressure-redistributing mattresses and cushions
  • Daily skin inspection
  • Adequate nutrition and hydration
  • Manage moisture (incontinence)
  • Avoid friction and shear

Treatment

  • Pressure relief is essential
  • Appropriate wound dressings by stage
  • Debridement of dead tissue
  • Treat infection
  • Nutritional support
  • Surgery for deep wounds
