Postherpetic Neuralgia

Postherpetic neuralgia (PHN) is chronic nerve pain that persists after a shingles rash heals. Risk increases with age and severity of acute shingles.

Features

  • Burning, stabbing, or aching pain in the area of prior shingles
  • Sensitivity to touch
  • Itching
  • Can last months to years

Treatment

  • Gabapentin, pregabalin
  • Tricyclic antidepressants (nortriptyline)
  • Lidocaine or capsaicin patches
  • Opioids in selected refractory cases
  • Nerve blocks

Prevention

  • Shingrix vaccine (recommended for adults 50+)
  • Early antiviral treatment of shingles
