Diabetic Nephropathy

Diabetic nephropathy is kidney damage that develops from long-standing diabetes. It is the leading cause of kidney failure in many countries but can be slowed substantially with tight blood pressure and glucose control.

Detection

  • Annual screening with urine albumin in everyone with diabetes
  • Serum creatinine and estimated GFR

Treatment

  • Tight glucose control (A1C target individualized)
  • Blood pressure control (target often <130/80)
  • ACE inhibitors or ARBs
  • SGLT2 inhibitors (strong kidney protective effect)
  • Finerenone for selected patients
  • Cholesterol management
  • Avoid NSAIDs when possible
  • Treatment of anemia and bone-mineral problems in advanced disease
