Diabetic nephropathy is kidney damage that develops from long-standing diabetes. It is the leading cause of kidney failure in many countries but can be slowed substantially with tight blood pressure and glucose control.

Detection
- Annual screening with urine albumin in everyone with diabetes
- Serum creatinine and estimated GFR
Treatment
- Tight glucose control (A1C target individualized)
- Blood pressure control (target often <130/80)
- ACE inhibitors or ARBs
- SGLT2 inhibitors (strong kidney protective effect)
- Finerenone for selected patients
- Cholesterol management
- Avoid NSAIDs when possible
- Treatment of anemia and bone-mineral problems in advanced disease
