Barrett's Esophagus

Barrett's esophagus is a condition in which the normal lining of the esophagus is replaced by tissue resembling intestinal lining, typically from chronic acid reflux. It slightly increases the risk of esophageal cancer.

Diagnosis

  • Upper endoscopy with biopsies (only way to diagnose)
  • Screening considered in long-standing GERD plus risk factors (male, white, over 50, obesity, smoking, family history)

Treatment and Surveillance

  • Proton pump inhibitors to control reflux
  • Surveillance endoscopies at intervals based on dysplasia status
  • Endoscopic ablation (radiofrequency, cryotherapy) for high-grade dysplasia
  • Endoscopic resection of nodular dysplasia
  • Lifestyle changes for reflux
